blur scan on iPhone in files

The iPhone simply cannot scan document because it gets blurry after saving.

Even with my 12 and my wife's iPhone SE.

Nothing helps before you start writing the restart and lates ios install... tried everything already!

Please solve the issue, it is clearly software related.
