Receiving calls saying User Busy, but Focus Mode NOT on

I just updated to iOs 15.1. People have been telling me they are being sent straight to voicemail. I was testing it with a friend after restarting my iphone (12 Pro) and when they called, I saw the banner at the top and it wouldn't let me answer and said "User Busy" but I deleted Personal Focus mode because it was jenky and would turn on randomly even though I had the settings turned off entirely for that one. And in this case, I have NO focus mode on at all.

I've checked all settings and the toggle on the side is not in DND mode and ringer is turned all the way up. What's going on with my phone? What other things can I check?
