Can’t add downloaded documents to Apple Wallet
Trying to add my Vaccine Record (image and QR code) to my Apple wallet. When I click the download button on the document there’s not an option to add to wallet.

Contact the Document issuer if they have "Add to Apple Wallet" enabled for their certificate? Probably not, that is why you are not seeing it.
You need to get in touch with the Certificate / Document issuer for this feature.
