How do you restrict child iPhone hotspot
I turned off hotspot on my child's phone and then went into Screen time and disabled Cellular Data Changes -- but he can still enable his hotpot.

Enabling restrictions for Cellular Data Changes prevents changes to whether an individual app can use cellular data or not, but doesn't affect Personal Hotspot. While there's not a Screen Time setting that controls Personal Hotspot, you can always send this suggestion to Apple using this link: Product Feedback
Since Personal Hotspot is a function that's part of your wireless plan, it may be worth reaching out to your wireless carrier to see if they have any options available.
