Apple care + no iCloud information

I have apple care but I cannot find my iCloud information to

file a claim. I have tried to locate my iCloud with no luck following different

steps. I need to file a claim because the device is lost. How can I do this, I

have the invoice, credit card and license to show proof I am the one that

purchased the device.

When you purchased AppleCare+, did you purchase the plan with Theft and Loss? If so, this resource should help:
