That's a good question. The first thing you can do is swipe up on the iPhone as if you were going to close all of your apps. This will show you the apps that are currently open, and it should be apparent which apps may be the culprit (ie. If Notes app and Instagram are open).
With iOS 15.1.1, you have to give permission for apps to use your microphone and camera. So, if you open the Settings app and tap Privacy > Camera, you'll see a list of the apps that have asked permission to use the Camera, and the switch will be green if you approved it, or white if you did not. Compare what you see here to the open apps mentioned earlier.
