Check the creation of the QR code. It may have no data the iPhone can use. What exactly does the QR code hold?
A URL? Small amount of text info? something else?

How are you attempting to read the codes that are having issues exactly?
Have tried a 3rd party QR reader and see if it works?
Where are these QR codes coming from? How are you trying to scan them exactly? What data are you expecting?
It’s not just 1 code and first time tried to scan codes
One is our business code so is active- trying to scan with camera and scans code and detects QR code but says no usable data
But it’s fine for hundreds of people who use our code
Solve what? If the QR code does not work, you would need to get a new QR code from whoever generated it.
There is no way to fix a QR code. It either works or it doesn't.
