I got a message saying that my iPhone has been compromised

I completed a survey from ATT which is my service provider. After filling it out I received a message that my phone had been compromised. Has this happened to anyone else? What should I do?

You did not complete a survey from AT&T. You completed a survey from a criminal attempting to steal your identity. You should contact the police and AT&T, and assume that any information you entered into the form has been stolen, and if there was any financial information you should contact the source for that information.

If you entered your Apple ID and password change the password immediately.

If you entered your AT&T user ID and password change that password immediately and contact AT&T.
