I am unable to save the recorded voice memo. I open the app, start recording, speak something, I can see the audio waveform in the window. When I press the stop button, the waveform stops but I don’t see any menu option to save it.
If I close the app and reopen, the recording is lost.
Note the screenshot attached. The done button appears disabled.

Uninstalling and reinstalling the Voice Memo did the trick.
Wondering if some small step is being overlooked ....
Eg: are you pressing the "Done" button when finished recording ?
I may be wrong ... if so, apologies.
Your iPhone image shows you still are in "pause" mode:
ie: is not "stopped" (finished), and you can only continue recording in that mode. ("Done" seems greyed out.)

I reported the issue on ios 15.1 but post upgrade to ios 15.2 the issue remains.
I have tried the links provided but do not solve my issue.
Post the recording all buttons on the screen are frozen, I cannot replay or save. Hence all the recommendations in the links do not work.
I have downloaded apps from the App Store for voice notes and they seem to work just fine. So the issue is not with the microphone. I have checked permissions for voice memo using microphone and that seems to be fine as well.

NB: (This is for iOS15, or open the article and change to your iOS version at the top for accurate info.)
Eg: says

The done button is inactive — essentially pressing it does nothing. Not sure why it’s inactive.

As I had mentioned that button is unresponsive. In fact on stopping all buttons become unresponsive - so I am essentially stuck. I cannot resume, stop or make a new recording. I cannot replay the recording as well.
Mmmm.
So I guess the other option is to Close and re-open the Voce Memos app, as you sat ...
... thus losing the unsaved recording, which you want to keep.
