No luck with normal start-up.

When I do the NVRAM reset I get the folder/question mark followed by a blank white screen followed by a sign in command which it takes followed by internet recovery followed by the utulity screen offering disk repair, time machine back-up which I have and operating system reload. None seem to help. Apple indicates my start-up disk or my Operating system is bad.

Ideas???

iMac, OS X 10.11

Sure sounds like the Hard Drive has failed and needs to be replaced.
