Why is it that when I have driving turned on my phone never rings, ever? Every call goes straight to voicemail. Started when I upgraded to iOS15.

It appears that the Driving Focus Mode is active on your iPhone and is activated as soon as your iPhone detects that you are moving in a car.
Open Settings ➔ Focus ➔ Driving: Set this to OFF.

When you manually activate the Driving mode to ON through Focus, then your iPhone is not a going to wait to sense whether you are driving or not. Once you manually activate it, then the iPhone assumes that you are driving and behaves accordingly.

Open Settings ➔ Focus ➔ Driving ➔ While Driving: Here, if you have it set to Automatically, then your iPhone will set it ON and OFF automatically when it detects that you may be driving. For this to work Automatically, you cannot have it set to Manually.
Furthermore, Open Settings ➔ Focus ➔ Driving: Here, if you are setting Driving to ON and OFF manually then the feature activates and deactivates when you toggle the switch ON and OFF.

Thanks for the reply Axel, but this isn't happening when I'm in the car, or moving. It NEVER rings, even when sitting on the table at home when the driving mode it on. The only fix I have is to turn off driving mode which I always thought was a great safety feature.
Thank you Axel. I will give this a shot. It's beginning to make sense though I have to say added "features" are not making things so that they "just work," as Jobs and Cook would like.

Unless I'm misunderstanding something then, it no longer senses whether or not I'm in a car, it's just a manual on/off switch. Sure seems like they are going backward on useablility here.
