If you did I think you would know unless someone else bought the AppleCare+ Theft and Loss policy on your behalf if it was a gift. As far as I know that requires a claim that is processed by the insurer, which is AIG. There's a deductible ($149), and it appears that the only way to receive a replacement is that it gets shipped by Apple after AIG processes and approves the policy claim. I also believe that when there's a lost or stolen claim, Apple will make sure that the replaced phone is bricked if it ever connects to the internet.

The OP will need the box, have the serial number being recorded somewhere. Another way is looking up the serial number through this via logging in and checking on Devices.

Unless you opted to purchase an AppleCare+ plan with Theft & Loss coverage, Apple will not replace your lost iPhone for free. You should know because this level of coverage would have added an additional $269 or more to the cost of the phone. Even with it, there is still a deductible charge.
To check to see what coverage you have go to Settings > General > About it will be shown just below the serial number.
