I don’t know why or how, but the Voice Memos app has gone glitchy for about 6 months by now. It’s both offloaded and downloaded at the SAME TIME. The name and icon are also strange. Here’s a screenshot.

Thanks for posting in Apple Support Communities.
We understand you have questions about the Voice Memo app being offloaded. You have the option to offload apps that aren't frequently used. For those apps, the app is removed from your device to preserve storage space, but the documents and data are kept intact.
If you decide you want to use the app, you can select the download button (cloud icon with down arrow). If you prefer not to use this feature, you can disable "Offload Unused Apps" in Settings > General > iPad Storage.
