To get the app back you need to download it from the App Store -> https://apps.apple.com/us/app/notes/id1110145109

Any Apple iOS or iPadOS default apps that can be deleted can be reinstalled from the App Store.

Then open the app and chose your iCloud folder from the folders tab.
