Select a PDF, press command-i, make sure open with is set to Preview. If not, change it to Preview then click the "Change all…" button.
Not certain, from above reply, if the computer is running Monterey 12.0.1 or if one is suggesting the solution is to update to version 12.1 will solve the issue.
It is alway a good idea to keep the computer up to date and 12.1 Monterey does bring Bug Fixes which maybe related to the Preview Issue.
Strongly suggest Making a Time Machine Backup is very useful and can be used to Revert to Previous Working macOS before updating to 12.1 Monterey.

Okay. Found it. Florida Dept of Revenue? It's a form you have to fill out I imagine. I would seriously consider installing Adobe Acrobat Reader DC.
FWIW, it is possible the PDF in question is not compatible with Preview. Do you have any problem with other PDFs?
Have no issues using Native Application of Apple called Preview to open any PDF File where created on this computer or from some other source.
Then again, do not have a Third Party PDF application installed on the computer.

No idea what a "FL DOR .pdf" is.
Have never had any issues using Preview to fill out Forms from Revenue Canada of any other Government source.
EDIT - they may have offered the link to Adobe to download but have declined the offer and still able to use Preview
I don't have a third-party .pdf app installed either and don't want to. I prefer Preview!
