1. Because it's tied to your Apple ID, and available to all units signed into it.
2. If you got an email receipt, use the 'Report a Problem' link in it. Otherwise, click here and ask the iTunes Store staff for a refund.

It's not being applied to any specific device, but to your Apple ID. To cancel it, click here and downgrade to the free 5GB.
