I have sound and volume but when I make a call I cannot put on speaker nor can I hear them and they cannot hear me

If the above options do not mitigate the issues then Contact your carrier and then Contact Apple Support

App Volume:

Are you using a third-party app for this (i.e. Zoom)? If so, increase the volume on the app itself.
