do you have to be above 18 in order to apply for the Apple Card, and can a ITIN number be used instead of a SSN ?

Yes -- 18 or older per the "See if you're eligible" section of How to apply for Apple Card - Apple Support

If a family member has an Apple Card they can add you as a "participant" if you are over 13.
