I just received an email notification advising me that I must cancel my Apple TV+ subscription before February 3 not to be billed for renewal.
I don’t recall ever subscribing or activating a trial. I cancelled the subscription through my iPhone but how can I find out when and how I became enrolled?

Apple had an offer for a free year of Apple TV+, with the purchase of an eligible device (iPhone, iPad, iPod touch, Apple TV box, or Mac), until last June. Apple has had a 3 months offer since. Did you, or anyone else with access to your account, possibly activate such offer on or around 2021-02-03 or 2021-11-03, possibly right after setup of a new device? After redeeming and signing up, the Apple ID user would have received a confirmation email with the subscription details.
Select third party companies may also have offered Apple TV+ with the purchase of a product or service, with their own duration and terms and conditions. That would still require knowingly redeeming.
Signing up requires the account password, Touch ID, or Face ID, for authorisation, so that it always has the user’s consent.
