No Meditation in the Mindfulness app on Watch
I have only Reflect and Breath in the app despite both OS on the iPhone and Apple Watch being up to date and I have a Fitness+ subscription with a number of meditations saved in the library on the phone.

I contacted Apple Support and ended up directly with the Fitness+ team. It appears that the problem was one that had yet to be reported but on an investigation with colleagues it seems that others had the Audio Meditations missing.
The fix was to open Fitness+ on the iPhone and start a meditation. A message briefly popped up regarding proceeding without the Apple Watch but the message was fleeting and it seems to have forced a connection to the watch. After that Audio Meditations appeared in the Mindfulness app on the watch.

Hey there Redsmartie,
Thanks for reaching out to Apple Support Communities. Make sure you’re following the steps from this article to access Fitness+ Audio Meditations:
Listen to guided Meditations on Apple Watch (Apple Fitness+ subscription required)
If that doesn't help, the next steps would be to unpair and pair your watch again and restore from the backup just created. This should result in no loss of data. It deletes all temp and corrupted files and re-indexes the file system.
Unpair and erase your Apple Watch
Take care.
In order to get meditations to show up on my watch, I had to power cycle my watch, open Apple Fitness+ on my phone, AND add a meditation to my library.
Hey Redsmartie,
If unpairing then pairing didn’t help, reach out to Apple Support at the following link so they can look further into this with you:
Take care.
I have a Fitness+ subscription and can see all content on my iPhone but not Apple Watch.
I have unpaired and erased the watch and set it up again but still I see only Reflect and Breath.
