Based on Apple’s article, please be sure you’ve followed these steps in the ‘Get help’ section to isolate the situation. 

If there is an adapter and cable involved, you’ll want to rule those out individually. Do you have another device you can attempt to use that set up with? Is the situation app specific or is nothing mirroring? Are you able to test other HDMI cables? Do you have a different monitor/TV to test the set up with?
