My iPhone is not paired with the car or any Bluetooth device. If it is plugged in to charge at home or in the car, no sound will come from the speaker. If I unplug the charger cord it immediately plays from the speaker.

Troubleshooting CarPlay and iPhone:
What to Try:
A. Pair in Park:
Try unpairing the phone. Then, pair it back. Perform this while in while in park.
B. View the Owner's Manual:
See what the Owner's Manual of your car says when you read it.
C. Some Links:
What are you using? If you're trying to charge from a USB port in the vehicle, that might connect to the audio system of the vehicle (where perhaps the volume is off). I've only seen those systems in newer rental cars, and the only way I saw around it was to use a lighter plug power adapter. There might be some other way.

How Is this Plugged In?:
Are you plugging this in to the DC port, using an adapter? OR are you plugging it in to a USB port of your car for syncing data? If using the DC Port, then Bluetooth would need to be enabled. If using a USB port, then Bluetooth does no need to be enabled.
So, if you need to use Bluetooth, then enable it, reporting back if it works. Be certain you pair it, as I mentioned in my first reply.
What are you using though? Are you plugging it directly into a USB port? Because automotive USB ports these days are designed for both power and data. Again, I've only experienced it with newer rental cars, where no sound came out of my iPhone once it was plugged in. If I had turn by turn directions, whenever there was a prompt, I would hear directions interrupting the radio.
And I never realized there was any difference between the two ports in the car that can be used to charge the iPhone. Thanks for that explanation.
I made a trip to the Apple Store and it was recommended I purchase a splitter. So, I’m plugging into the USB port in the car with the charging cable which I plug into the splitter that enables me to either plug headphones in for audio or leave that port open. If I leave it open the speaker on the phone works! If I am using Mapps as an audible aid when driving, The phone pauses the booker podcast I’m listening to in order to give driving directions. So, a solution for me was to purchase the splitter.

Most cars these days have one that directly interfaces with the interface. I've even seen a few where if I was using Apple Maps, it would show the direction right on the car's display.
But if you have the volume off on the display system, you wouldn't hear anything.
