Thanks, saw that but when I click on that link doesn't take me anywhere helpful. Looks like it needs to be sent in but no information as to how to set up a claim.
Did you click the blue link (contact Apple Support)? When you see words in blue in an article, you click the words to take you to the link about how to do what you need.
