Missed Calls when answering

Hi,

I have an iPhone 12 Pro Max with iOS 15.3.1 installed. For a while now I have been unable to answer incoming calls.

Whenever the phone rings, I slide to answer the call and the phone moves to the call waiting screen, showing two incoming calls from the same number?

No matter what I do, whichever button I press nothing happens. I have to let the call ring out and then I end up with a situation like this in my call history:

I have tried the following:

Restarting
Toggling airplane mode on/off
Resetting to factory settings
A new SIM Card
Disabling Wi-Fi calling
Disabling call waiting
Disabled calls on other devices
Disabled call forwarding

I'm rapidly losing patience at this point as I can't answer any calls!

Does anyone know what's happening or have a fix for this?

One thing we wanted to suggest is if you had call forwarding enabled or set up, it would be a good idea to contact your carrier to confirm there isn't anything on their end.
