If you select the Monthly Apple Care+ option, and your Apple Card is set as the primary payment method for your Apple ID as explained here: link-> Change, add, or remove Apple ID payment methods - Apple Support
Then it will use your Apple Card to pay for the Apple Care+ monthly charge each month.
If you are buying the one time purchase version of Apple Care+, then choose your Apple Card during checkout.
