Cec enabled/Apple TV menu item “off”

What do I do if I have cec enabled but the “controls tvs and receivers” menu item is “off” and greyed out?

Apple TV HD, tvOS 12

I do have CEC enabled on all components (Apple TV, Denon receiver, and LG tv).

Also, yeah that’s a dated tag. Apply tv is os 15.4.

For what it’s worth both the remote and the remote app used to turn up the volume on the receiver, and a couple months ago, stopped. No idea why. Was hoping it would resolve after the next update and so far no change.

Huh... Flakey cable? Unplug everything for a minute or two.

What happens if you plug the ATV into another HDMI input on the receiver? I assume the TV and receiver have an [e]ARC HDMI connection? Does it work if you plug the ATV into the TV instead of the receiver (or the receiver instead of the TV)?

Have you tried resetting the ATV remote (don't see why that would grey the ATV's CEC switch, but...)? If your Apple TV remote isn't working - Apple Support

If your ATV's Control TVs and Receivers option is off and greyed out, my guess is that whatever you're connected to is not ready to play nicely with HDMI CEC. This could be as simple as a setup option on your TV, soundbar or AV receiver. As @bruno75 said, more details about your setup would be more likely to elicit useful suggestions.

Oh - I also see that the footer under your post reads "Apple TV HD, tvOS 12". If that's still true, you should update your tvOS to the newly-released v15.4.
