Do you always transfer the sim card from the old phone to the new to complete activation?

You do not necessarily have to, especially if you purchase your iPhone from your carrier. Also, if you purchased a new iPhone that supports 5G, your carrier may require you to get a new SIM that supports 5G, or they would activate your eSIM.
