I have an iPad from my doctor at the va but it doesn’t have App Store this is my first apple device as well. Is there away for me to remove restrictions. My doctor doesn’t mind this as long as I make my video appointments.

I doubt that you will be able to do that. My guess is the device is managed by the VA and all restrictions are handled by Mobile Device Management. It has the App Store restricted for a reason and won't let you log in with your own Apple ID for using the App Store.
