I get the following error message: Storage system verify of repair failed. :(-69716).
I have a iMac (Retina 5K, 27-inch, 2017).
Aside from not being able to use the latest version of two programs because they require later OS than my Mojave 10.14.6, I am experiencing no other computer issues.
In reviewing the "fix Fusion drive" link provided by BDAqua, I see the following:
"If your Fusion Drive appears as two drives instead of one in the Finder, it's no longer working as a Fusion Drive. Here's how to fix it."
"If you see a drive labeled Fusion Drive, your Fusion Drive is working and this article doesn't apply to you."
My McIntosh HD is labelled "Fusion Drive". I.E. My Fusion Drive does NOT appear as two drives = "...your Fusion Drive is working and this article doesn't apply to you".
Do I still need to perform what the How to fix a split Fusion Drive - Apple Support link instructs?
Error 69716 from Monterey Update
There are reports appearing on the ASC Forums regarding failed installation of Monterey on older Qualifying Apple Computers. In most cases this is related to having a NON Apple Original Internal Drive ?
The EFI Boot ROM included in Monterey is verifying for Original Apple Drive and if not found , may Balk or fail to upgrade to Monterey.
Reports indicate the EFI BOOT ROM is Required to Flash the Computer and is a requirement for a Successful Monterey Installation.

on an iMac (late 2012) with a 1TB Fusion Drive.
anyway, long story short... needed to recreate the fusion drive from the recovery terminal (after Backup, of course). After recreation, installation worked fine.
This is definitely a Fusion hardware issue. I think that the drive is bad and needs to be replaced.

Try to update by starting in safe mode or update to Big Sur then try to do Monterey. You can get update Bog Sur from here:

I downloaded DriveX. Here's the report on my iMac HD. Everything looks good.
I called Apple support (800-275-2273). The tech recommended I install Big Sur or Catalina and then Monterey. Big Sue is no longer available. I'm downloading Catalina now.
My iMac 27" has ONLY the original hard drive that came with it.
I performed a Mac disk utility First Aid & repair while in safe mode (command-R) this AM. Snapshot 1 of 5 showed the following:

old 5829970 / oxid 12072419
snapshot is invalid
volume /dev/rdisk3s1 could not be verified completely
File system check exit code is 0.
Restoring original state found as mounted.
Operation successful.
I also, while in safe mode, re-installed Monterey 10.14.6 successfully.
I am still unable to upgrade Monterey 10.14.6.

Something appear not right with the Internal Drive
If willing try DriveDx to drill down on the Internal Drive and see if it finds the issue. Could be signs of drive failure
Although Catalina installed and is working fine, I just tried to install Monterey and got the message: Storage system verify of repair failed. :(-69716)
I'm fine for now, but as my software programs get updated, they will require Monterey.
Advice on how to get Monterey installed will be greatly appreciated.

I have been able to update my iMac's OS to Catalina 10.15.7. I'll attempt to update to Monterey tomorrow.

I at first tried to get Big Sur but found out it is no longer available. I then downloaded Catalina and installed it. Installed fine.
Think you need to Fix the Fusion Drive as suggested by @ BDAqua and instruction provided in that posting.
Nope, at least not because it broke apart.
I am unable to upgrade to Monterey from Mojave 10.14.6.
