Apple Watch No Notifications or Heart rate

Apple Watch Series 3 paired with iPhone 12 Pro max.

No longer gathering heart rate data outside of a workout

No longer prompting stand ups

No longer notifying daily goals completed

No longer notifying monthly goals completed

No longer notifying me of a call or message (whatsapp or imessage)

I have tried all the re-sets, the re-starts, the unpairing, the pairing, the restore from backup, the off and the on. Please do not suggest any of those.
