A couple of weeks ago I received a call from a supposed cash app rep from customer support offering help with my account due to suspicious activity. They sent me a link via text to gain access to my cash app app. By clicking that link and approving access to the app, did I give them access to my actual phone too and not just the app?

If all you did was click the link and open a browser - w/o providing any personal info - probably not.

However, "giving them access" might indeed be more problematic.

If it installed an APP (which it sounds like it might have) delete the app and review the installation and permissions of ALL of your apps.

Grant "ALWAYS" access sparingly and ONLY when really needed.

Consider changing your password(s) for the "targeted" application(s).

That said, consider this a learning experience ... we've all dodged these sorts bullets from time to time.
