My Insurance on my phone to send in

My phone has been shutting down and restarting for a couple of months now , doesn’t allow my back camera doesn’t work and my audio is very low . I wanna know how I can go about sending it in and also would I be sent a replacement in place of the phone because I have work and school and a lot of information is sent to me through my phone
