Every time I start playing a song, or pause and resume a song (after more than several minutes or so), the volume plays twice as loud as my desired setting. The volume indicator level (squares) do not change during this--it's just the output. This happens from the MacBook speakers or when using external speakers.

I disabled the “Sound Check” option under Music > Preferences > Playback menu.
I haven’t had the issue since.

Start up your Mac in safe mode and see if the issue continues: How to use safe mode on your Mac - Apple Support Then, depending on the outcome, follow the correct steps for whether or not the volume issue continues in safe mode.

Shut down or restart your Mac - Apple Support
Also, it looks like you have macOS 12.3 tagged in your post, if that is the version you have installed currently, you'll want to update to the latest version of macOS Monterey which is currently 12.4.

It's good to know this only happens in the Music application. In the test user account, you will need to sign in to your Apple ID account to bring your music library over and fully test this issue: Access your music library on all your devices - Apple Support

A test user account was only able to access Safari and the problem doesn't happen with audio playing from Safari. It is ONLY happening in the Apple music app and happens with streaming music or playing a selection from my downloaded library.
I was able to set up another user and and tested this the same way. No change--the volume still starts loud and after ~2 seconds, reverts to the lower volume.
I am running OS12.4 and restarting the laptop doesn’t solve the issue.
Unfortunately, the issue continued even in Safe Mode.
This happens to me as well, it's very annoying.
