I forgot to remove my SIM card from my Apple 6s before packaging it up and sending to Apple for recycling. What should I do now?

Please contact your carrier and disable the SIM plan and get another SIM as a replacement.

Go to your carrier and ask for a new SIM card, assuming you have a subscription (not prepaid) and that you can prove ownership.
