How does mail app work when at top of email there is question asking if you want to be unsubscribed from subscription
"Unsubscribe? ”this message is from a mailing list”
And mail will try to unsubscribe your email.
What protocol does the mail app use to do this? Thanks

In general, mailing list blasts contain an unsubscribe link. Mail parses the emails, identifies that link, visits the page and if needed autofills your email address in the form and submits it, all in the background.

Do you know if any further monitoring occurs, or is each individual removal matched to the individual email it was a originally requested from?
In other words, is the request a one time only occurance, then it disappears?

AFAIK, it's a one-time action taken for just that sender. How effective it is depends on the sender, and whether or not they actually comply with requests to drop you from their mailing list. No doubt some senders will do so, but they simply sell your email address to other lists. 🤬
