To take out your SIM Card, use the provided SIM Card ejector that came with your iPhone. Stick the tool into a small hole on the side of the iPhone and press until the SIM tray comes out.

Regarding your second question: The reason that may happen is because you have no iCloud storage left or the two phones were disconnected during transferring process.
