If my phone was stolen and the find my iPhone app location notifies me when the phone is turned on can I get a history of the times and locations of the phones find my iPhone was activated?

No, but if the phone is off, you might get a single "last known location" for a limited time.....and you might not.
Realistically, sorry but there is about a zero chance that you will ever see the phone again.

Be very careful if you receive messages or emails that the phone has been "found" and you need to provide some information so the phone can be returned to you. They may look genuine, but these are scams that are trying to trick you into providing your Apple ID and Password so the bad buys can get into your phone.
