You can't transfer AppleCare to another device. You can either transfer it to a new owner of the old phone as described in Transfer your AppleCare plan - Apple Support or you can cancel it as described in Cancel or change the payment method for your AppleCare plan - Apple Support and purchase a new AppleCare plan for the new phone.
