Using a hotspot
T Mobile said I can use a hotspot and avoid paying a monthly fee. Is that true. My apple watch uses GPS.

Yes, your Apple Watch should be able to connect through Wi-Fi to the personal hotspot from your phone. If it's connected through bluetooth, it will still have some internet access.
