Are you looking at your phone when you swipe down to access the control center? If so that is the issue if you have face id enabled.
Lay the phone down next to you and try swiping down and see if it opens or not.

If it only worked once you need to erase your device and set it up as new. your software is corrupt. (not hacked or compromised)

the phone unlocks quickly if you are looking at the screen so its really hard to distinguish which is which.
I tried the force reboot once and it did work but only once. I may try the reset all setting. Thanks for your suggestions, much appreciated.
Thanks for your feedback. Instead of erasing and given the fact that IOS 16 is coming, do you think an update to 16 will fix the issue? Thx.
