Storage System Verify or Repair Failed. :(-69716)

I was trying to upgrade my Mac Book Air from Mojave to Big Sur. The installation fails with the above error. If I run Disk Utility while booted normally, First Aid tells me that "First Aid found corruption that needs to be repaired. To repair the startup volume, run First Aid from Recovery. Click Done to continue." If I reboot in Recovery Mode and run Disk Utility as instructed, First Aid tells me everything is fine. If I then reboot in regular mode and try to continue the installation, I get the same error. Any suggestions?

on an iMac (late 2012) with a 1TB Fusion Drive.

anyway, long story short... needed to recreate the fusion drive from the recovery terminal (after Backup, of course). After recreation, installation worked fine. 

This is definitely a Fusion hardware issue. I think that the drive is bad and needs to be replaced.

I do not have a fusion drive. I have an internal 500GB Mac flash drive. Since the system appears to work ok and it is just the upgrade that fails, I would surmise that only type of hardware failure fitting this scenario would probably be bad blocks/sections of the SSD. Does anyone know of further diagnostic and/or repair techniques such as a hardware error log?

That looked useful, so I downloaded and ran it. It did not find any errors. It did show 76% wear leveling, which knowing how SSDs work is not a concern for a 7 year old SSD drive. All of which, which leads me to conclude that my issue is definitely not a hardware failure and is instead some form of software structure corruption that needs repair. Any other suggestions? Thanks!
