Included the results from diagnosis in additional text, saw an issue on this forum from a while back of someone helping out another in troubleshooting slow iMac and he used these diagnostics. Not sure what to do now since i was not able to reply comment on that original post. Im having an issue with my iMac (late 2013 21.5" i5) running slow and getting random pop ups.

Unsigned files - There are unsigned software files installed that could be malicious and should be reviewed.
You should run Malwarebytes for Mac to clean up any malware:
Malwarebytes for Mac - Malwarebytes.
After running Malwarebytes, you can uninstall it:
Uninstall Malwarebytes for Mac - Malwarebytes Support.
Adobe Flash Player installed - Adobe Flash Player is installed on this computer. This is a security risk and no longer supported.
As EtreCheck says, Adobe Flash is no longer used nor supported, and should be deleted:
Uninstall Flash Player for Mac OS - Adobe Support.
No Time Machine backup - Time Machine backup not found.
Make sure you have a backup of your Mac. Having a backup ensures your data is protected from accidental deletion and when something goes wrong.
With Time Machine, you can easily create a backup of the files on your Mac. To start your first Time Machine backup, you need an external hard disk. If you don't already have one, these specific external hard disks, sold by Apple, are excellent options and compatible with Time Machine:
If you have an iMac earlier than 2017, or a Mac mini, you will also need a USB-C to USB-A 3.1 cable, such as this one, for compatibility with your older generation Mac. So, for your iMac (21.5-inch, Late 2013), you'll need the adapter cable.
You can then Back up your Mac with Time Machine - Apple Support.
Additionally, you can use iCloud to also backup your Mac. iCloud can act as an off-site backup, but iCloud should be used in addition to Time Machine. Here are some resources on setting up iCloud for Mac:
After you setup iCloud, turn off Optimize Mac Storage in System Preferences > Apple ID. That way, all changes made to files in iCloud are also backed up to Time Machine.
Other than that, the reason your iMac is slow is due to the 5400-rpm mechanical hard disk drive. While this hard drive found in your Mac is perfectly suited to light tasks, such as email, web browsing, etc., for more advanced tasks, and for users who want the maximum performance from their Mac, this hard drive does have its limitations. Luckily, you can use an external SSD as your startup disk to run macOS and all your data from that. It will make your Mac 10 to 30 times faster for storage, and in turn, make your entire Mac system much faster. However, what I would do, and another great option, is to replace this Mac with a new Apple Silicon Mac: Mac - Apple. A new Apple silicon Mac would be much faster than your current Intel-based Mac, and be fully up to date, unlike yours which is no longer supported for support and updates.

What app is that report from? That isn't an EtreCheck report, which is what we need.
EtreCheck is a useful App to diagnose and identify issues, both hardware and software. It is trusted by many users. Please follow the steps outlined below:
1. Navigate to: https://etrecheck.com and get the free version of EtreCheckPro.
2. Download it. It is a free and trusted app. It is often recommended here on Apple Support Communities.
3. Open EtreCheck, and before running the report, be sure to check “Allow full Drive Access” at the bottom of the EtreCheckPro window.
4. Run the report, and put it in a post, as shown here: https://discussions.apple.com/docs/DOC-250000211. We will then examine the report for issues in your Mac.

sounds good, just ran the report on etrecheck, here are the details.
Thank you!! Everything is back to normal operating speeds now, main issue was malware. Once I downloaded Malwarebytes and it did its job everything worked out.
