Remove widget iOS 16

Just installed iOS 16 and somehow added a black round empty widget; can’t seem to remove it and am following all instructions available.

Does it look like this?

If yes, you have enabled Assistive Touch.

To turn AssistiveTouch on or off quickly, triple-click the side button (on an iPhone with Face ID) or triple-click the Home button (on other iPhone models).
