No. You cancel the AppleCare plan on your current phone and buy a new plan for the new phone. If you paid for the plan on current phone up front, Apple will give you a prorated refund of the unused portion of the plan you didn't use. If you pay for the plan monthly, you'll just stop paying for the plan on the old phone once cancelled and then start paying for a plan on the new phone.
