The Apple TV app itself is bundled with macOS 10.15 Catalina or later. It is not available for earlier versions system software.
The Apple TV+ streaming service should be available in the browser at tv.apple.com for prior versions macOS.

iMac 27" 5K (Late 2014, Mid 2015) is upgradable to macOS 11 Big Sur.
iMac 27" 5K (Late 2015 or later) is upgradable to macOS 12 Monterey or later.
