I found this answer to address my issue,
"The Mail App privacy preferences redirect remote requests through two independent relays.
If you want to click the button for those, disable "Protect Mail Privacy" and enable "Block All Remote Content."
but I cant find these mail app privacy buttons to check/uncheck

In Mojave... Mail>Preferences>Viewing>Load Remote content in messages. - yes i have this box checked but it doesnt make any difference - i think maybe the sender didnt attached images correctly

Mojave has no "Protect Mail Privacy" and enable "Block All Remote Content", Monterey or iOS15 needed I believe. :)
It's possible they didn't exist in Mojave. I no longer have Mojave installed to check.

Most frequent mistake is including the location of the image on their system instead of the actual image.
