my wife’s phone (12 max pro) says screen of

why is it different? Is there different meaning to this? Is it different based on the phone?

The thing is I have always on display turned off. It’s been turned off since I got the phone.

I have an iPhone 13 Pro Max and an iPhone 14 Pro Max in the same household. I can confirm that the iPhone 13 Pro Max states "Screen OFF" while the iPhone 14 Pro Max states "Screen IDLE". Yes, the iPhone 14 Pro Max had Always ON mode set to OFF as well.

The moral of this experiment is that the Screen OFF and IDLE are different based on the iPhone model regardless of whether Always ON is set to ON or OFF.
