Apple notes hashtags greyed out

Apple notes hashtags greyed out

I noticed that Some of the Hashtags I've created in the Apple Notes app are greyed out and removed from the Hashtag list. This happens regardless of which device I’m using. here are some of the situations when this happens:

When I close and reopen the Notes app on my Mac.
When I flip though apps on my mac.
When i turn off and turn on mac.
When I close and reopen a note with Hashtags on my iPhone.

Please help me fix it and thanks for reading this.

My devices:

iPhone 11 (IOS 16.0.2)
MacBook Pro late 2015 (MacOS Monterey 12.6)

Restart both devices to see if the issue is still there.
Update both devices to rule out the older software.
