Can you launch/open the Apple TV app? Else delete and re-install it from the Roku Channel Store.
Can you sign in, or are you still signed in to the Apple TV app? Go to the in-app Settings (sprockets wheel icon) and check the that you are signed in with your Apple ID account. After that, go to subscriptions management, to see if Apple TV+ is still listed as active for that account.
Did you redeem the Apple TV+ offer from T-Mobile as they described? The service is not automatic. Users have to redeem and sign up of the service, to link it to an Apple ID account.

Yes I am sorry I need to know how to get the apple tv back. I have a years worth and would like to be able to watch it. Right now I have logged in on my Roku and the service is locked up.
