An "AccessibilityUIServer" missing icon in iCloud settings within iOS 16.1.
In iOS 16.0 appeared a "web" missing icon in background apps list
And for many years a mysterious "Keyboard Data (Chinese...)" missing icon in the backup details. If anyone wonders, no I have not used a Chinese keyboard ever as far as I recall.
Hope this helps to get them fixed.

Did fill out a feedback report!..Hope Apple gets them fixed. I also saw new missing icons on the apple watch app afterwards; for screen time, game center and tracking notifications under show notifications on wrist down.
What are the answers to the issue listed by josep70 ?

If those icons need changing, apple will need to do it.
